Below are a few items you will need to bring with you during the Arrangement Conference.  This information is needed to help create the Death Certificate for your loved one:

 

  • Full Name of the Deceased

  • Date of Birth of the Deceased

  • Social Security Number of the Deceased

  • Address of the Deceased

  • Last Occupation of the Deceased

  • Highest Level of Education

  • Place of Birth

  • Marital Status

  • Maiden Name (If the deceased is a female)

  • Father's Name

  • Mother's Name and her maiden name

  • Military Service

  • Branch Rank

  • Insurance Policy Number and beneficiaries Information (if arrangements will be made through an insurance company)

Additional Information and Affiliations

  • Fraternal organizations

  • Service organizations

  • Union memberships

  • Any special recognitions

  • Gather personal information for your obituary

  • Choose burial or cremation.

  • Select which photographs and memorabilia you’d like to display.

  • Consider any customs, traditions, or religious rites that are important to you and your family.

  • Choose the clothing you’d like your loved one to be dressed in.

  • Select music and hymns, if desired.

  • Together we can select a casket or cremation container.

  • Together we can help you to choose locations and types of services.

  • Together we can help you to choose floral arrangements.

  • Together we can help you to choose a theme for decorations and stationery.

  • Together we can help you arrange transportation for your family.

  • Together we can help you select a burial plot or mausoleum.

  • Decide who will keep your urn if loved one is being cremated.
